HOUSING PROGRAMS
There are times that housing can be a roadblock for individuals achieving the success they desire. ChangePoint continues to develop a number of housing programs designed to help individuals at many different levels of care receive the extra support they need to achieve long-term success and independence.
ChangePoint has independent, semi-independent and fully supervised housing programs in Show Low, Pinetop-Lakeside, Holbrook and Winslow.
Independent Housing is currently for individuals with an Serious Mental Illness (SMI) diagnosis that may just need some additional support and assistance from a case manager. Rent is based on a tenants’ income.
Semi-Independent Housing, currently located in Holbrook consists of 5-two bedroom and 6-one bedroom apartment homes. This housing is for those clients who are on AHCCCS and have an Serious Mental Illness (SMI) or General Mental Health (GMH) diagnosis. ChangePoint staff is available onsite during daytime hours to help empower tenants with living and socials skills in order to help them achieve long-term independence.
ChangePoint now has a fully supervised group home housing program in Pinetop-Lakeside that is aimed at serving those with an Serious Mental Illness (SMI) or General Mental Health (GMH) diagnosis and are struggling with living independently. Staff is available to the residents 24/7 and work with them on life skills, socialization, nutrition and a number of other skills related to activities of daily life to help the residents move to independent living and find long-term success.
