Heath Lindsey, PA is a nationally certified physician assistant specializing in primary care. Heath completed his bachelor’s degree in Fine Arts from Arizona State University. Later in life, he decided to go back to school and received his Master of Physician Assistant Studies from Texas Tech University. Heath was born in Tucson, Arizona but was raised here in the White Mountains. Heath provides primary care at ChangePoint Integrated Health, along with being an on-call medical officer for Change Point Psychiatric Hospital. Heath strives to provide the best possible care for his patients by utilizing knowledge, compassion, listening skills and collaborating with other medical professionals.
Annie Oakes, PMHNP-BC
Annie Oakes is a board certified Psychiatric Mental Health Nurse Practitioner. She completed her Masters of Science in Nursing at Walden University and her Bachelors of Nursing from Brigham Young University-Idaho. Annie has been in the healthcare field for over 14 years and believes in a person-centered therapeutic approach to include both prescriptive medicine and therapy modalities to help patients achieve their mental health goals and improve their quality of life.
Tara Iacono, PA-C
Tara Iacono is a nationally certified physician assistant specializing in psychiatry and addiction medicine. She graduated with honors from the St. John’s University Physician Assistant program in 2006. Tara is an active advocate for her profession in the PA community and is a member of the American Academy of Physician Assistants and the Arizona State Association of Physician Assistants. She is also on the committee of the AAPA Special Interest Group: PAs in Virtual Medicine and Telemedicine. Additionally, she is pursuing her doctorate in healthcare administration and leadership at Rocky Mountain University Health Professions. Paul Tilyou, MD is a graduate of Louisiana State Medical School in New Orleans. Dr. Tilyou completed his Psychiatry Residency at the University of New Mexico in Albuquerque. Dr. Tilyou sees patients as a General Psychiatrist. Dr. Tilyou has lived and worked in Arizona for over 20 years and has worked with Changepoint Integrated Health since 2014.

Dr. Abdallah is a triple board-certified in General Psychiatry, Consultation-Liaison (Psychosomatic) Psychiatry and Addiction Psychiatry. Dr. Abdallah is experienced in treating psychiatric illness in people with complex medical/psychiatric conditions and is an expert in addiction medicine. He is passionate about quality care.

He continued training in a general psychiatry residency at Banner Good Samaritan Medical Center in Phoenix Arizona. Then Dr. Segal completed a forensic psychiatry fellowship at the University of Southern California, Keck School of Medicine in Los Angeles. Dr. Segal is double board-certified in General and Forensic Psychiatry. Dr. Segal is the Chief Medical Officer at the Valley Hospital in Phoenix, Arizona.
